Submit via e-mail, fax or mail (i) any company’s application, (ii) an ACORD general liability application plus any company’s contractor’s supplemental application, or (iii) a WESTCAP Standard Application.

The principal reasons we need to request additional information before we quote:

(1) Incomplete five year loss history.

(2) Expiring terms were not provided.

(3) Incomplete application, especially gross receipts and description of
     operations.
